This is a very tiny but serious problem. When picking up the 'Axe' in UGX Requiem, it's not possible to switch to the axe anymore due to... an "unbound" control?

The button "X" is used for that, I spammed it countless amounts of time, but still, no results.

This also happens with the suit in Leviathan, I'm not able to equip it as well, which limits my experience with the map. Same goes to requiem as well.

Sounds to me like you were playing around with the controls and you unbound the actionslot or bound to to something else.
I suggest you go into your controls and set them back to default as that's what most maps are designed for. If you switch your controls to something else, its on you to remember what button it is set to.

Turns out I was just messing with the combat controls. A quick default settings reset fixed the problem.
