Does someone know how I can make my own Custom perk machine??

First I want to know If I can make the 3D model with 3dsmax or do I need another program? and then I want to know how can I make it( I mean how to make the script for it in which format i have to save the 3D file and things like that) or if there is another post where they explain it please leave me the link

Must be transferred to either maya or blender to export to the game.
