Title: How do I install a map?

I'm really interested in playing the Perish map for Zombies on BO1 but I have absolutely no idea how this works. I installed the two links that came with it, including the game_mod. If someone could give me a little tutorial on what to do, that would be amazing. Thank you for all the hard work.
